Job Description

As a Healthcare Case Manager, you will assess patient needs, coordinate care plans, and collaborate with interdisciplinary teams to ensure patients receive timely, compliant, and holistic healthcare support while navigating complex care pathways.

The Qualifications We Seek

• 1 to 3 years of relevant experience in healthcare case management or a related role

Your Daily Tasks

• Patient Assessment: Conduct thorough assessments of patients’ medical, psychological, and social needs to determine appropriate levels of care and services.
• Care Planning: Develop individualized care plans in collaboration with patients, their families, and the healthcare team to ensure needs are met.
• Care Coordination: Facilitate communication between healthcare providers, patients, and external services to ensure timely and coordinated care delivery.
• Monitoring & Evaluation: Monitor patient progress, evaluate the effectiveness of care plans, and adjust them as needed to achieve optimal outcomes.
• Resource Connection: Assist patients in accessing specialists, home care, and social support services, and help navigate the healthcare system.
• Patient Advocacy: Act as an advocate for patients by ensuring access to appropriate care, services, and support while addressing barriers to care.
• Compliance: Ensure all care coordination activities comply with state, federal, and organizational regulations, including HIPAA.
• Education & Support: Provide patients and families with education on healthcare options, treatment plans, medications, and long-term care management.
• Documentation: Maintain accurate, timely, and comprehensive documentation of patient care, case management activities, and communications.
• Risk Management: Identify high-risk patients and intervene with appropriate resources to prevent complications and ensure continuity of care.
• Collaboration: Work closely with interdisciplinary teams, including physicians, nurses, and social workers, to deliver holistic care and resolve complex cases.
